Join PSA as an INSTITUTIONAL MEMBER

Please follow the application procedure outlined below:

 

1. You can apply online, or you can download and accomplish the application below.

 

2. If you prefer to download the application form, submit accomplished application form to the

    PSA secretariat. You have 4 options to submit your application form to the secretariat: (1)

    personally, (2) scanned copy may be forwarded by email to psa.sec@gmail.com, (3) Fax to  

    456-1928, or (4) thru the liaison officer of your agency, if any.

 

3. Pay the application fee equivalent to the annual membership fee to the PSA Secretariat, or

    thru PSA Authorized Banks or thru the PSA Liaison Officer. If you wish to pay thru the bank,

    please contact the PSA Secretariat for the account number

 

4. Download the details of payment form below and submit accomplished form to the PSA

    Secretariat personally, by fax or e-mail or thru the PSA Liaison Officer in your Office or

    organization, if there is one. If payment is made thru the bank, please provide the PSA

    Secretariat a copy of your validated deposit slip.

 

5. Upon receipt of the above documents by the PSA Secretariat, you will be issued an official

    receipt for your payment.

 

6. Once your application has been approved by the Board of Directors, your application fee

    serves as your annual membership fee for the year when membership is approved and you

    will be given an access code for the Member’s only section.

 

7. An orientation of new members will be scheduled and you will be given your PSA ID.
